You will need a copy of your driver’s licence to obtain your claims history statement (see below) if you request it by mail.

If you make your request by mail, it is important to carefully follow the instructions. In particular, the form must have your original signature and be accompanied by a photocopy of your driver’s license (free/delay up to 20 days).
